The Photo ID can be found in the Dollar Store in the City for only $10. As for the enrollment form, it goes between Ken finding it and giving it back to you when you ask him and Nathaniel having it mixed in another file.

Found in Classroom A on the first desk to the right behind the chair with no desk to it.
